Word Version of the Evaluation Form
The following directions are for using the Word version of the evaluation form which completes calculations.

COMPLETING THE PERFORMANCE PLAN SECTION OF THE EVALUATION FORM: |

- TAB to go from one field to another.
- The Continuation Sheet is located at the end of the evaluation form. (Use only when due to lack of space on the evaluation form. Please note: Assigned points listed on the continuation sheet are not reflected in the assigned points total on pages 1 and 2 of the evaluation form. A total of 100 points must be determined and distributed between the job duties in all four categories.)
- Once the Performance Plan portion of the form is completed, print this portion of the form. (Remember to print the continuation sheet if used.) Employee, evaluator and next-line supervisor sign the Performance Plan in Red ink.

COMPLETING THE FIRST AND SECOND INTERIM REVIEW SECTION OF THE EVALUATION FORM: |

- After the completion of each Interim Review Meeting, evaluator must print the Interim Meeting Documentation to obtain the dates and signatures of the evaluator and the employee in Red ink.

COMPLETING THE ANNUAL EVALUATION CALCULATIONS ON THE EVALUATION FORM: |

- To determine ratings, tab over to the determined rating. Click on the determined rating and enter the rating number to indicate rating chosen. (Example: If you are giving the employee a rating of 3 for a particular job duty /responsibility, you would click under the appropriate rating field and enter a 3.)
- Requires that all the calculations of page 2 be transferred to the original evaluation form which contains the employee's and supervisor's signatures/dates as the original evaluation was developed with the intent that all ratings and math calculations were to be placed in Section I of the evaluation form.
- The rating calculations on page 1 of the Performance Plan and the continuation page can be printed and submitted as the original as there are no original signatures required on this page.
PLEASE NOTE: After completing the annual evaluation, only submit the completed four (4) pages of the evaluation form containing the original signature, ratings and the continuation sheet to your agency's central personnel office.
Employee Performance Evaluation Form (MSWord - 361 KB) - If you have trouble running/enabling/accessing Macros on this form, go into Tools and change the Macro Security to Low. Close the form and then re-open it.
