Last Name
REQUIRED FIELD – Should automatically populate from the Last Name field in the “Contact Information” section.

Middle Initial
Enter the first letter of your middle name.

Suffix

Do You Have a Valid Commercial Driver’s (CDL) License?
A “commercial driver’s license,” or CDL, is a license that certifies you to operate commercial vehicles, such as heavy trucks and equipment, buses, and large passenger vans.

Indicate whether or not you currently possess a valid commercial driver’s license. A “valid” commercial driver’s license means it currently is not listed as revoked or suspended by the Department of Motor Vehicles due to traffic violations, criminal violations, or any other violation.

If Yes, Expiration Date
If you indicated that you do currently possess a valid commercial driver’s license in the previous field, indicate the month, day, and year the license is set to expire as of now. This expiration date may be found on the actual license.

If Yes, What Class?
Commercial driver’s licenses are issued for a certain class, meaning the bearer of the license has been certified to drive the particular vehicles. If you indicated that you currently possess a valid commercial driver’s license, indicate the class of the license.

If Yes, What Endorsement?
Commercial driver’s licenses often have endorsements for specific types of commercial vehicles. If you indicated that you currently possess a valid commercial driver’s license, indicate any endorsements listed on the license.

Has Your Driver’s License or CDL Ever Been Revoked or Suspended?
If you indicated that you currently possess a valid driver’s license or commercial driver’s license, indicate whether or not it has ever been listed as revoked or suspended by the Department of Motor Vehicles due to traffic violations, criminal violations, or any other violation.

CONVICTIONS

In this section, applicants are asked questions regarding their conviction history, if any exists. Applicants must provide a complete list of all convictions of any crime, other than a minor traffic violation. Applicants must provide: (1) conviction; (2) date of conviction; (3) county of conviction; and (4) state of conviction.  Applicants must list all convictions on every application even if they have been previously disclosed.

A conviction is not an automatic rejection. However, an applicant may be rejected for failure to provide an accurate list of all criminal convictions. If an applicant does not know all of the required information of a conviction, or if the applicant is unsure whether they have recalled each conviction, the applicant should obtain a copy of their criminal background record prior to submitting the application. For more information on requesting Kentucky criminal records, go to the Administrative Office of the Court's website at http://courts.ky.gov/aoc/courtservices/recordsandstatistics/records.htm. To request criminal records from other states, an applicant should contact the appropriate court in that state for more information.

Criminal background records will be requested and reviewed by the Personnel Cabinet to ensure accurate reporting by the applicant.

The Personnel Cabinet will review an applicant’s criminal background history and determine if the applicant should be eligible for employment in that particular position, pursuant to KRS 18A.032 and 335B.020.

Have You Ever Been Convicted of Violating Any Law Other Than Minor Traffic Violations?
REQUIRED FIELD – “Convicted of violating any law” includes convictions for misdemeanors, felonies and violations. Such convictions include, but are not limited to, the following: trespassing, theft, assault, harassment, contempt, driving under the influence (DUI), any conviction involving alcohol or drugs, etc. Minor traffic violations should not be included. Minor traffic violations include, but are not limited to, the following: speeding, parking violation, non-moving traffic violation, etc.

A plea of “guilty” (including an Alford Plea) entered in court or a finding of “guilty” by a court is a conviction.

Diversion Program: An applicant who is currently enrolled in a “pretrial diversion program” for a crime is deemed to have entered a conditional plea of “guilty” to the crime. An applicant should list the conviction on the application and indicate that the applicant has entered a diversion program.

Expunged Conviction: An applicant who has had a misdemeanor or violation expunged by a court of law is not required to include the conviction on the application, nor does the applicant need to mark that they have been convicted of violating any law. However, if there are additional convictions which have not been expunged, an applicant must include each of these additional convictions.

If you answered “Yes” to ever being convicted of violating any law, each conviction must be listed (one conviction per field). Minor traffic violations should not be included (minor traffic violations include speeding, parking violation, etc.).

WORK AVAILABILITY

Each vacant position posted has a specific type of work, shift, and location. When an agency posts a position, they are looking for a candidate who is available to work under these particular conditions.

Date Available for Work
REQUIRED FIELD – Indicate the earliest date you will be available for work if selected for a position. If currently available, select today’s date.

Type of Work
REQUIRED FIELD – Select the type(s) of work for which you seek to be hired. Multiple responses may be selected by holding the Ctrl key while selecting each response.

  • Full-time – at least 37.5 hours per week
  • Part-time – less than 37.5 hours per week
  • Interim – only for nine (9) months or less (could be full-time or part-time hours)

Shift Availability
REQUIRED FIELD – Many positions work the same shift every workday. However, some positions rotate shifts, meaning there is not necessarily a consistent shift schedule; these positions may also rotate days and hours of work.

Select the shift(s) you would be willing to work. Multiple responses may be selected by holding the Ctrl key while selecting each response.

VETERAN’S PREFERENCE STATUS

When an eligible list (Register) is certified to a state agency for employment consideration, all candidates who qualify for Veterans’ Preference will be appropriately identified.

Effective with registers certified on July 21, 2008, an employing agency shall offer an interview to at least five (5) individuals identified on a register as entitled to Veterans’ Preference, including current state employees.  If there are fewer than five (5) individuals entitled to Veterans’ Preference on the register, the employing agency shall offer an interview to all individuals who qualify for Veterans’ Preference.

The following individuals may qualify for Veterans’ Preference, once the appropriate documentation is supplied to the Personnel Cabinet:

Former and Current Members of Active Military, Military Reserves, or KY National Guard:

  • Copy of discharge, DD 214, or current military ID

Spouse of Disabled Veteran:

  • Copy of discharge or DD214, plus
  • Statement from VA Benefit Rating Board showing that spouse’s present disability is service-connected, plus
  • Statement from treating physician that affirms veteran cannot perform the duties of his/her usual occupation

Un-remarried Spouse of Deceased Veteran:

  • Copy of discharge or DD214, plus
  • Proof of veteran’s death while on active duty or proof that veteran’s permanent and total disability is service-connected

Dependent Parent of Deceased or Disabled Veteran

  • Copy of discharge or DD214, plus 
  • Proof of veteran’s death while on active duty or proof that veteran’s permanent and total disability is service-connected

If you are approved for a particular classification based on meeting the minimum requirements via your education and later appointed/hired to a position with that classification, the Personnel Cabinet must receive educational verification within ninety (90) days of your appointment.

Verification of Foreign Education Credentials
All education used to approve an applicant for a title’s minimum requirements must be verified and submitted to the Personnel Cabinet. The Personnel Cabinet does not recognize foreign education unless a U.S. college, university, or academic accrediting agency determines that it is equivalent to degrees or courses offered at an accredited U.S. school or institution. Verification will only be accepted from organizations accredited by the following academic accrediting agency:

VOCATIONAL / TECHNICAL / BUSINESS EDUCATION and/or TRAINING

Many degrees, certificates, and diplomas can only be earned at a special instructional institution, like a vocational, technical, or business school. Such specialized education or training is not offered at universities or colleges. Examples of these areas of education include but are not limited to welding, auto mechanics, word processing, etc.

Upon applying to a vacant position, you should provide original, certified transcripts or other official means of education verification for the education you indicate on your application to the Personnel Cabinet. If you are approved for a particular classification based on meeting the minimum requirements via your education and later appointed/hired to a position with that classification, the Personnel Cabinet must receive educational verification within ninety (90) days of your appointment.

List the name of the most recent instructional institution where you have earned any vocational, technical, business education, or training credit hours and/or a degree, certificate, or diploma.

APPRENTICESHIP

Apprenticeship is a training system that produces highly skilled workers in a particular trade. The apprenticeship concept is that on-the-job training is supplemented with technical classroom instruction. Apprentices work under the supervision of qualified journeyworkers to develop their chosen trade or skill and learn the techniques, materials, and equipment associated with that trade. Classroom instruction that provides training in theoretical and technical aspects of the trade is required.

If you have completed an apprenticeship program, complete this section, even if it is not related to the position to which you are currently applying.

Enter the name of the trade area in which the apprenticeship certificate was awarded.

Name and Address of Apprenticeship
Enter the name and address of the program sponsor. A “sponsor" means any person, association, committee, or organization in whose name or title the program is registered, irrespective of whether such an entity is an employer.

Select the month, day, and year you were awarded the apprenticeship certificate.

Length of Program
Select the period of time that most closely reflects the scheduled duration of the apprenticeship program.

OCCUPATIONAL LICENSES / CERTIFICATIONS

Many classification titles require certain licenses and/or certifications (if so, this requirement will be listed under the “Special Requirements” section of the Classification Specification for that title). It is to your advantage to complete this section even if the title you are currently seeking does not require an occupational license or certification.

Upon applying to a vacant position, you should provide copies of the licenses and/or certifications you indicate on your application to the Personnel Cabinet. The Personnel Cabinet must receive verification of your license(s) and/or certification(s) pertinent to the classification title prior to appointment.

Do not list your driver’s license or any commercial driver’s license in this section; this information should be listed in the appropriate fields in the PERSONAL INFORMATION section of this application.

KY License or Certification Title
List the title of your occupational license or certification. All licenses and certifications must be issued by a Kentucky licensing agency.

Indicate the license number found on your license or certification.

Name, Address and Phone of Licensing Agency
List the name, address, and phone number of the licensing agency that issued your license or certification. Please include the area code. Use hyphens accordingly. Separate the name, address, and phone number by commas.

Original Issue Date
Indicate the month, day, and year the license or certification was originally issued to you. This date should be stated on your license or certification.

Current Expiration Date
Indicate the month, day, and year the license or certification will expire. This date should also be stated on your license or certification.

License Information

EMPLOYMENT HISTORY

An applicant’s employment history is important in determining whether or not the applicant meets the minimum requirements for a classification title. In this section, applicants should list all of their employment history, beginning with their first job. When completing this section, please list all positions you have ever held, even if one or more of your previous positions do not directly relate to the position to which you are currently applying. Any employment history added at a later date that does not appear on a previous application but occurred before the date of that application must be verified by the employer. It is absolutely to your advantage that all work experience is listed.

List details as accurately and thoroughly as possible about each work experience. Any new information not included on a previous application but is in regard to a job listed on a previous application must be verified by the employer.
